Quinapril Overdose-Induced Renal Failure
نویسندگان
چکیده
Background: Angiotensin converting enzyme (ACE) inhibitor toxicity is infrequently reported and seldom produces significant clinical effects. Only 15 previous cases of intentional ACE inhibitor overdoses among adults have been published. The most commonly observed clinical effect is hypotension, which is typically transitory and responsive to supportive measures and intravenous hydration. An additional 48 pediatric exposures were reported in a single case series, none of which resulted in any adverse effects related to the ACE inhibitors. Reversible impairment in renal function has been reported in 5 cases of ACE inhibitor overdose, but only in association with systemic hypotension (systolic blood pressure < 90 mmHg). We report a case of intentional quinapril overdose where the patient presented two days later with acute renal failure in the absence of clinical features of systemic hypotension. Additionally, this is the first reported case of quinapril overdose of which we are aware.

Seizure and Rhabdomyolysis: Serious Complications of Tramadol Overdose
To the Editor; Tramadol, a synthetic opioid is mainly used to treat acute and chronic pain. Although this drug is not an over-the-counter medication, it has been illicitly accessible to and abused by opium addicts (1-3). Studies showed excessive and widespread use of tramadol in Iran (1-4). Tramadol may cause constipation, drowsiness, tachycardia and respiratory distress, while the most severe...
متن کاملHyperkalemia, renal failure, and converting-enzyme inhibition: an overrated connection.
Hyperkalemia is widely viewed as a common complication of ACE inhibition in azotemic patients. These renal failure patients are the patients who benefit most from ACE inhibition. Because we could not confirm this notion after a retrospective evaluation of 236 azotemic patients, we studied 2 models of renal mass reduction. In the first, we did a 5/6 nephrectomy (Nx) on rats and studied them 2 we...
متن کاملACE inhibitor versus beta-blocker for the treatment of hypertension in renal allograft recipients.
Angiotensin-converting enzyme (ACE) inhibitors have been shown to slow the progression of chronic renal failure. However, the value of ACE inhibitors for the treatment of hypertension in renal allograft recipients has not been established. ACE inhibitors dilate the efferent glomerular arteriole, an effect that may aggravate the decrease in glomerular filtration rate resulting from cyclosporine-...

متن کاملFrequency of renal impairment in paracetamol overdose compared with other causes of acute liver damage.
In an analysis of 160 patients with fulminant hepatic failure, the frequency of renal failure in the patiens who had taken an overdose of paracetamol was not significantly higher than in those with hepatic failure due to other causes. The same was found in another group of patients with less severe hepatic damage. In both the latter group and the patients with fulminant hepatic failure, the dev...
